REQUEST FOR

PROPOSALS

Vendor to fulfill Pre-employment Background Screening needs for

Dakota County

NOTICE is hereby given that sealed proposals will be received by Dakota County until 4:30 p.m. on Friday, January 13, 2012 in the Dakota County Employee Relations office, 1590 Highway 55; Hastings, MN 55033.

Proposals are being requested for an experienced vendor to fulfill the organization’s pre-employment background screening needs in accordance with the specification provided in the Request for Proposals.

The selected provider will be responsible for the following:

1.Academic Verification

oVerification of highest level of education

2.Driving Record Screen/Verification

oVerification of valid driver's license

oAny driving violations that have occurred in

the past 5 years - nothing beyond 5 years

3.Employment Verification

oVerification of all employers listed by the

applicant for the past 5 years.

oVerification should include Job Title, Start

Date & End date.

4.Criminal History

oCriminal history screen should be done for

all counties the applicant has lived within the

past 7 years.

5.Psychotherapist Verification

oIncludes verification of any violations of the

MN Stat 143A regarding Psychotherapists for

all relevant employers that the applicant was

employed at during the past 3 years.

6.Credit

oA report outlining the credit history of the

applicant to include: current and previous

debts, payment amounts, late payments,

garnishments,and past due amounts and other

related information on every credit source the

individual has used.
