NOTICE OF REQUEST FOR

PROPOSALS

Pre-employment Medical Screening and Occupational Health Clinic Provider for

Dakota County

NOTICE is hereby given that sealed proposals will be received by Dakota County until 4:30 p.m. on Friday, January 6, 2012 in the Dakota County Employee Relations office, 1590 Highway 55; Hastings, MN 55033.

Proposals are being requested for a Medical Provider to provide pre-employment medical screenings and occupational health clinic services in accordance with the specification provided in the Request for Proposals.

The selected provider will be responsible for the following:

1.Conduct and process DOT and non DOT Drug and Alcohol Screenings

2.Conduct occupational health designated functional pre work screenings.

3.Conduct designated pre placement exams

"Back evaluation

4.Conduct respirator medical evaluation

5. Administer and read 2-step mantoux test

6.Administer and read 1 step mantoux test

7.Administer Hepatitis B 3-dose series vaccination

8.Provide medical treatment for work related employee injuries.
